The monetary value of an RVU is determined by the annual conversion factor. The 2021 Medicare conversion factor, as defined in the Medicare Physician Fee Schedule final rule, is $32.4085. This means Medicare will pay $32.4085 per RVU in 2021.
On the downside, CMS set the 2022 conversion factor (i.e., the amount it pays per RVU) at $33.59, which is $1.30 less than the 2021 conversion factor.

A major medical procedure requires more skill and more time, so it will generate a higher RVU. The cost of maintaining or operating a medical practice: These factors include rent costs in addition to the price of equipment and supplies. Doing a similar exercise for Code 99214 would result in a Total RVU of 5.10930.
How many RVUs is Botox?
The CPT code for the injection of Botox, 52287, has a work relative value, RVU, of 3.20 and total RVUs of 9.16 in the office and 4.90 in hospital or ASC.
Total RVUs: Total RVU combines three types of RVU: work RVU, practice expense (PE) RVU, and malpractice (MP) RVU before arriving at a final RVU value. MP RVU is a reflection of professional liability insurance cost based on the risk associated with a CPT code.

Medicare assigns 4.58, 4.89, and 3.26 Relative Value Units (RVUs) to “Welcome to Medicare,” “Initial Annual Wellness Visit,” and “Subsequent Annual Wellness Visit” respectively. In comparison, it assigns 2.05 RVUs to a 99213 level follow-up visit and 3.01 RVUs to a 99214 level follow-up.
A two-view chest procedure has an RVU of 0.62, whereas a two-view ankle series has an RVU of 0.48. (RVUs can be obtained from the CMS Web site at www.cms.hhs.gov.) Radiology managers can derive productivity benchmarks by researching the RVU for each procedure.
These are best demonstrated by an example. For echocardiography (American Medical Association Current Procedural Terminology [CPT] code 93306), there is a technical component of 3.75 RVUs to pay for staff time, disposables, and equipment depreciation necessary to perform the procedure.
Based on 2018 AMA CPT designations, a 12 lead EKG interpretation (CPT code 93010) generates 0.24 RVUs.
The Specialty Society Relative Value Scale Update Committee (also known as the RUC) determines the RVUs for each new code and revalues existing codes on a five-year schedule to reflect changes in costs and technology.
How many RVUs does an assistant surgeon have?
In other words, the primary surgeon would receive 20.72 wRVUs and the assistant surgeon would receive 3.32 wRVUs.
Under CMS's proposal, in 2021, 30 minutes of psychotherapy done with an E/M service (90833) would involve less work (1.50 work RVUs) than 30 minutes of psychotherapy done independent of an E/M service (90832, 1.70 work RVUs).

Procedures are also assigned RVU values. A joint injection might be worth three quarters of an RVU while a vasectomy is 11 RVUs.
Remember that both the amount of drug used for injection and the drug wasted should be charged. For example, if a 200-unit vial of onabotulinumtoxinA is mixed to inject a person being treated for chronic migraine with 155 units, the 45 units wasted are also billed.
Total RVU is simply the sum of the three component RVUs described above. It is multiplied by the Medicare Conversion Factor, a national number set annually that converts the RVU into a dollar amount.
RVUs and established patient visits

By better documenting just a few charts on every shift, a physician can significantly increase his the total RVU production. Conversely, don't waste time and effort over-documenting simple, straight-forward medical issues. Make sure to do a full procedure note – site, size, technique, complexity.
What is an RVU example?
For example, a 99213 (office/outpatient visit; established patient) has a work RVU of 0.97. A 99203, the same level for a new office visit, has a work RVU value of 1.47, meaning that it is assessed about 50 percent more time and effort than the 99213.
